Form Test

I have read, understand and agree to follow Harrisburg Christian Performing Arts Center’s 2021 Trunk or Treat rules.

I shall indemnify and hold Harrisburg Christian Performing Arts Center harmless from any liability imposed upon Harrisburg Christian Performing Arts Center, adjudicated or otherwise, by virtue of any personal injury, illness or property damage arising from any use of the premises on the Harrisburg Christian Performing Arts Center by me, my agents and invitees, or by any acts done thereon by me, my agents and invitees, including court costs and counsel fees.

Media Release Authorization: I understand that by participating in this event at Harrisburg, I give Harrisburg Christian Performing Arts Center permission to photograph or video me/my child, my guests and my vehicle publicly to promote the Harrisburg Christian Performing Arts Center. I understand that the images may be used in print publications, online publications, presentations, websites, t-shirts and social media. I understand that no royalty, fee or other compensation shall become payable to me by reason of such use.
